Privacy Statement for Active and Former General Motors Employees Participating in the LifeSteps Program
(LifeSteps Privacy Statement)
This privacy statement is being provided to let active and former General Motors (GM) UAW-represented, IUE-CWA-represented, and salaried employees (Employees) know how their health information is handled by General Motors in the LifeSteps program.
Collection and Use
Participation in the LifeSteps wellness program is voluntary. You may choose to complete and return a Health Risk Appraisal (HRA) online, (http://www.lifesteps.com/gm/Atoz/default.jsp) at health fairs, or on forms mailed to your home. When HRA's are collected online and at health fairs a GM supplier collects the individually identifiable health information from the HRA, and submits it to the University of Michigan's Health Management Research Center (University of Michigan). The University of Michigan maintains the individually identifiable health information that you submit on your HRA. GM suppliers are required to use this individually identifiable health information solely for the purpose of administering services in the LifeSteps program. The following are examples of individually identifiable health information collected on the HRA:
- Blood type
- Weight
- Blood pressure
- Cholesterol level
The University of Michigan uses the information from your HRA to provide you with important information about your health. For example, the University of Michigan may provide you with individually tailored health recommendations based on the health profile identified in your HRA. Your de-identified/aggregated health information may be used to provide important statistical information to GM and the UAW. This information is useful because it helps GM provide programs and services to better serve the needs of employees. GM does not have access to the individually identifiable health information that you provide on your HRA. However, nothing in this privacy statement prevents the use of the legal process by GM or other parties.
Information Security
GM recognizes the importance of safeguarding individually identifiable health information and GM suppliers are obligated to take reasonable steps to protect your individually identifiable health information from unauthorized access or use. It should, however, be recognized that no technical, physical, or administrative safeguards can ever perfectly protect personal information.
Accessing and Updating Personal Information
Employees may access certain personal information to check it for accuracy. Employees are responsible for keeping their personal information up to date where GM has made systems or processes available for Employees to do so.
Updating Personal Contact Information - To update or correct personal contact information in the LifeSteps program you may contact the Manager of GM Corporate Health Programs at (313) 665-1630.
Updating Health Information - To update individually identifiable health information for the LifeSteps program, for example, weight, blood pressure or cholesterol level, Employees must complete a new HRA and submit it to the University of Michigan.

No Contract
This Privacy Statement does not constitute a contract between GM and any Employee, and it does not modify an Employee's month-to month or day-to-day employment relationship.
Changes
GM reserves the right to change this LifeSteps Privacy Statement. Any such changes will be published so that they are accessible to affected Employees.
This statement is effective as of February 25, 2004.
